Terms and Conditions
Effective date: 10/16/2026
About these terms
These Terms and Conditions govern use of the website operated by Elevate Chiropractic LLC, operating as Elevate Chiropractic (“we,” “us,” or “our”). Please read them before using the website. By using the website, you agree to these terms to the extent permitted by applicable law. If you do not agree, please discontinue use.
These terms address website use. Separate intake documents, treatment consents, financial policies, and any other agreements you enter into with the practice govern the matters they specifically cover. These website terms do not replace those documents or reduce rights that applicable law protects.
Educational information and professional care
Website content is provided for general education about chiropractic care and our practice. It is not a diagnosis, an individualized treatment recommendation, or a substitute for an examination and advice from an appropriate health professional. Do not delay obtaining professional care because of something you read or watch here. Consult a qualified professional before starting an exercise or treatment described on the website.
Reading the website, sending a general inquiry, or requesting an appointment does not by itself establish a doctor-patient relationship or mean that we have accepted responsibility for your care. A treatment relationship is established when the practice accepts you for care or as otherwise provided by law. This does not alter an existing treatment relationship or our legal duties to a patient.
Chiropractic treatment is recommended only when clinically appropriate. An evaluation may result in recommendations for another provider, additional testing, or a different course of care.
Emergencies
This website and its forms are not emergency services and are not monitored continuously. For a medical emergency, call 911 or seek immediate emergency care. Do not wait for a website, email, or text response.
Appointments and contact requests
Our booking links direct you to the practice’s ChiroTouch scheduling service. Use accurate contact information and book only for yourself or someone you are authorized to assist. Follow the booking service’s confirmation instructions; a general contact-form submission is not an appointment confirmation.
Appointment availability may change. Contact the office if you have not received confirmation or need to change a visit. A booking does not guarantee a particular procedure or treatment; care depends on the clinical evaluation and appropriate consent.
Fees and payment information
Current fees and the services included in them are described on our Pricing page and in the financial information provided by the office. Confirm the applicable fee and any offer conditions before receiving care. Changes in posted prices apply prospectively and do not override an existing agreement or applicable disclosure requirements.
For ordinary self-pay visits, our website describes an out-of-network model with payment collected after treatment and superbills available on request. A superbill does not guarantee insurance reimbursement. Benefits and eligibility depend on your plan and circumstances. Contact your insurer directly about reimbursement questions.
Services connected with a work injury, auto accident, personal-injury claim, or government insurance program may involve different rules. Confirm the applicable payment arrangement with the office before care. A website description does not guarantee claim approval, insurance coverage, payment by another party, or a particular legal outcome.
Any cancellation, missed-appointment, deposit, package, or refund terms will be provided separately before they apply. These website terms do not themselves create a cancellation fee or authorize a charge.
Results and testimonials
People respond differently to care. We do not guarantee a particular result, relief within a set number of visits, or that treatment will be appropriate for every condition. Testimonials describe individual experiences and are not a prediction of your results. Any clinical recommendation will depend on your circumstances.
Nothing in these terms prevents you from posting an honest review, making a lawful complaint, or exercising a protected right to discuss your experience.
Responsible website use
You may use the website for lawful purposes. You must not:
-
Submit false appointment requests, impersonate another person, or provide information you are not authorized to disclose.
-
Attempt unauthorized access to accounts, patient information, administrative systems, or other restricted resources.
-
Introduce malicious code, interfere with security controls, or disrupt website availability.
-
Use website forms to send unlawful, threatening, or abusive material, spam, or unsolicited advertising.
We may restrict website access reasonably necessary to address misuse or security concerns, subject to applicable law. Restrictions on website access do not remove rights you may have as a patient or replace appropriate procedures for continuity of care.
Website content and intellectual property
The website’s original text, design, logos, photographs, and other materials are owned by the practice or used with appropriate permission, except where another owner is identified. You may view the website and print or download reasonable portions for personal, noncommercial use while keeping applicable ownership notices intact.
Other copying, distribution, or commercial use requires permission from the relevant owner unless permitted by law. These terms do not restrict lawful linking, ordinary search-engine indexing, fair use, or another use protected by law.
Privacy and information you submit
Our Privacy Policy explains website information handling. Please use general contact forms for administrative inquiries and follow their privacy instructions. Do not submit medical records, payment-card information, or other sensitive information through a general form or ordinary email; ask the office for the appropriate channel.
By submitting a message, you permit us to use it to respond and address the request, subject to applicable privacy law. You do not grant us a general right to publish your identity, health information, photograph, or message for advertising. These terms are not a HIPAA authorization or consent to marketing calls or texts.
External services and links
The website includes links or resources supplied by other organizations, including ChiroTouch scheduling, maps, and social media services. Their own terms and notices may apply when you use their services. Review them before providing information or completing a transaction.
We do not control every external service or guarantee its availability or accuracy. This provision does not disclaim responsibilities we have for vendors acting on our behalf, protected health information, or obligations that cannot lawfully be excluded.
Website availability and accuracy
We aim to provide useful, accurate information, but website content may contain errors or become outdated. We may correct content, update features, or temporarily interrupt access for maintenance or other operational reasons. Contact the office to verify information that is important to an appointment or care decision.
To the extent permitted by law, the website is provided on an “as is” and “as available” basis, without warranties of uninterrupted access, error-free operation, or fitness for a particular purpose. This disclaimer concerns the website and does not replace professional standards of care or excuse a misleading representation.
Limits and rights that remain protected
To the extent permitted by law, we are not responsible for indirect or consequential losses arising solely from the unavailability or technical interruption of the website. This limitation applies only to ordinary website use.
Nothing in these terms excludes or limits liability for clinical care or professional negligence, a breach of confidentiality, fraud, intentional misconduct, gross negligence, or any responsibility that cannot legally be limited. These terms do not waive patient rights, consumer rights, or the right to contact a regulator.
Maine law and disputes
Maine law governs these website terms, except where applicable federal law or another mandatory legal protection controls. Any court proceeding concerning these terms must be brought in a court with appropriate jurisdiction in Maine, unless applicable law gives you a right to use another forum. These terms do not require arbitration or waive a right to bring an eligible small-claims case.
You are welcome to contact the office about a concern, but doing so is not a required step before exercising a legal right.
Updates and interpretation
We may revise these terms and will post the revised version with an updated effective date. Changes apply prospectively. Where applicable law requires additional notice or affirmative agreement, we will obtain it. Posting a revision does not change an existing treatment or financial agreement without any consent required by law.
If a provision is found unenforceable, the remaining provisions continue to apply to the extent permitted by law. A delay in enforcing a provision does not by itself waive the right to enforce it later.
