Privacy Policy

Effective date: 10/16/2026

Who we are and what this policy covers

Elevate Chiropractic LLC, operating as Elevate Chiropractic (“we,” “us,” or “our”), provides chiropractic care in Topsham, Maine. This Privacy Policy explains how we handle personal information collected through the website displaying this policy and through general website inquiries.

Health information may receive additional protection under Maine law and, where applicable, the Health Insurance Portability and Accountability Act (HIPAA). Those protections apply based on the information and circumstances, including when information is submitted through a website. This policy does not replace a patient Notice of Privacy Practices or authorize disclosures of protected health information. Contact our office for information about patient-record privacy and any applicable Notice of Privacy Practices.

Information you provide

Our contact form requests your first and last name, email address, phone number, and message. We collect the information you submit and correspondence needed to respond. If you contact us for another person, provide only information you are authorized to share.

Please use the general contact form for administrative questions. Do not send medical records, detailed symptoms, Social Security numbers, payment-card details, or other sensitive information through that form or ordinary email. Call the office to ask how to send sensitive information through an appropriate channel. Website messages are not monitored continuously. For an emergency, call 911.

Technical information

When you load a page, our hosting systems and providers delivering website resources receive technical information needed to send that page or resource to your device. This can include your IP address, browser and device information, requested resource, referring page, and access time. Providers may maintain logs for service delivery, security, troubleshooting, and system administration.

How we use information

We use information collected through the website to:

  • Respond to your questions and follow up on your requests.

  • Help arrange appointments and communicate about scheduling or administrative matters.

  • Operate, maintain, troubleshoot, and protect the website and our systems.

  • Keep appropriate business records and meet applicable legal obligations.

  • Send optional practice updates or promotions when permitted by law and consistent with your communication choices.

Submitting a general inquiry does not, by itself, enroll you in automated marketing texts. Any use of health information remains subject to applicable health-information rules and required permissions.

Online appointments through ChiroTouch

Our Book Appointment links take you to our booking page hosted by ChiroTouch. Information you enter there may be processed by ChiroTouch for us and made available to our practice for scheduling and patient administration. Review any notices displayed within the booking service before submitting information.

Our practice remains responsible for the patient information handled on our behalf under applicable law and our vendor arrangements. ChiroTouch’s public corporate privacy policy is not a substitute for our patient privacy notices. Contact our office with questions about your appointment information or patient records.

Maps and external website resources

The contact page uses OpenStreetMap map tiles and loads software resources through the jsDelivr content delivery network. When your browser requests those resources, the providers receive technical information such as your IP address and browser request details. This can occur when the contact page loads, without clicking an external link. Their processing is described in the OpenStreetMap Foundation Privacy Policy (opens in a new tab) and jsDelivr Privacy Policy (opens in a new tab).

The website also links to Google Maps and social media services. If you follow an external link, that service’s privacy notice applies to its own collection and use of information. Posting information publicly on a social media or review platform can make it available to other people. Do not post health information that you wish to keep private.

Cookies and tracking choices

Cookies are small files stored on your device. Similar technologies can also collect information about website activity. The following disclosure describes the technologies actually used on this website, their purposes, and the choices available to you.

Cookies are small files stored on your device that websites and online services can use to support functionality or remember preferences.

Our website uses OpenStreetMap to display our office location and jsDelivr to deliver website resources. When these resources load, your browser connects to the providers’ servers and sends technical information, which may include your IP address, browser information, referring website, and requested resource. These connections can occur automatically when you visit the contact page, even without clicking a link. Information about these providers’ practices is available in the OpenStreetMap Foundation Privacy Policy (opens in a new tab) and jsDelivr Privacy Policy (opens in a new tab).

You can manage or delete cookies through your browser settings. Restricting cookies may affect features that rely on them. Blocking cookies does not necessarily prevent technical information from being transmitted when your browser requests maps or other external resources.

If you have questions about website technologies or your privacy choices, contact Elevate Chiropractic at 207-406-2428.

Your browser generally lets you block or delete cookies. Blocking cookies may affect features that depend on them, and it does not necessarily prevent technical information from reaching providers when external resources load. Contact us if you need help understanding the choices described above.

When information is disclosed

We may provide information to service providers that help with website hosting, maintenance, form processing, communications, scheduling, or security, as needed for their work and subject to applicable contractual and legal protections. Our staff and authorized service providers use information for the purposes described in this policy.

We may also disclose information when you direct us to do so, when disclosure is required or otherwise permitted by applicable law, or when lawfully necessary to address fraud, security incidents, or threats to safety. A request for information does not automatically permit disclosure of health information; any additional authorization or legal process required by health-privacy law still applies.

If ownership or operation of the practice changes, relevant information may be transferred as part of that change only as permitted by law and with applicable confidentiality protections. The cookie and tracking disclosure above addresses any sale of personal information or sharing for targeted advertising; ordinary service-provider arrangements are described separately here.

Email and text communications

We may respond using the contact information you provide. Tell us if a number or email address changes or if you need a different way to communicate. Ordinary email and text messaging can be accessed by others and may not be suitable for sensitive health information.

You may unsubscribe from promotional emails using their unsubscribe link or by contacting the office. For any text program you separately join, follow its opt-out instructions or contact us. Opting out of promotions does not cancel an appointment or prevent necessary administrative communications through an appropriate channel. Any permission required for a marketing program is obtained separately.

Retention and security

We retain information for as long as reasonably necessary for the purposes described here, taking into account the reason it was collected, whether an inquiry remains active, applicable recordkeeping requirements, and legitimate needs to resolve disputes or maintain security. Clinical records follow applicable medical-record retention requirements. A deletion request may not result in deletion of information we are legally required or permitted to retain.

We use reasonable administrative, technical, and physical safeguards appropriate to the information we handle. No internet transmission or storage system can be guaranteed completely secure. Contact the office if you believe information submitted to us has been misused or exposed.

Your requests and choices

You may contact us to ask about information we hold about you, request access or a correction, request deletion, or change communication preferences. We will evaluate your request under the laws that apply to the information and our practice, verify your identity when appropriate, and explain any lawful limitation on the response. Where an applicable law provides an appeal right, we will explain how to exercise it.

Patient-record requests may follow a separate process and may have different rights, deadlines, and retention rules. Please call before sending identity documents or medical information so we can direct you to an appropriate channel.

Children and geographic scope

This website is intended for adults seeking information and for parents or other authorized people arranging care. It is not directed to children under 13. If you believe a child has submitted personal information through the general website without appropriate permission, contact us so we can address it under applicable law. Information collected in providing pediatric care is handled through the practice’s clinical procedures, with any applicable rights of minors preserved.

Our practice is located in Maine, United States. Online providers may process technical information in the United States or other countries. Using the website does not waive any privacy rights that apply to you.

Policy updates

We may update this policy when our practices or applicable requirements change. We will post the revised policy and update its effective date. If a change requires additional notice or consent, we will provide that notice or obtain that consent before applying the change as required. An update does not retroactively authorize a disclosure that was not otherwise permitted.

What Our Patients Say

Waiting area at Elevate Chiropractic with chairs, a sofa and an ALIGN neon sign

I have been seeing Dr. Minni since he was in Lewiston. We decided to follow him to Topsham, because of the amazing care me and my wife have both received. He is so thorough, and really cares! From the moment you walk in the door you feel like family. Can’t wait to see his practice grow! For all of the skeptics out there about Chiropractic care, I was there with you, until I tweaked my back just right and could barely walk. He was able to do some quick adjustments and I instantly felt some relief. Within a few days it was even better. It does take some time, and don’t expect instant results as it comes with time.

If you are in the Topsham area give him a call! Remember it’s not just backs/spine. Any joint can be manipulated!

Also, if you don’t like manual manipulation, that’s ok too, he has an actuator that he can use instead. He actually uses a little of both for us, and it works really well!
